NYC Home Rehab Physical Therapy PLLC provides one-on-one, patient-focused physical therapy in our outpatient clinic. We specialize in evidence-based rehabilitation to improve mobility, strength, balance, pain, and overall function for patients of all ages—especially post-operative, neurologic, orthopedic, and senior populations.

Our therapists deliver customized treatment plans that may include therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training, post-surgical rehab, fall-prevention, and functional mobility retraining. Whether in-clinic or in-home, every session is tailored to support independence, safety, and long-term health.

We proudly serve Astoria, Long Island City, Manhattan, and surrounding NYC areas. Medicare and commercial insurance with out-of-network benefits accepted.

Noreen De los Reyes, PTA

Noreen De los Reyes is a licensed Physical Therapist Assistant in New York with clinical experience delivering high-quality rehabilitative care in outpatient orthopedic, occupational injury, and multidisciplinary rehabilitation settings. She works under the supervision of licensed physical therapists to support patients recovering from work-related injuries, accidents, and a wide range of neuromusculoskeletal conditions.

She completed her Physical Therapy education at Cebu Doctors’ University in the Philippines. She is licensed as a Physical Therapist Assistant in New York State and is also a licensed Physical Therapist in the Philippines, providing her with a strong international clinical foundation and a comprehensive understanding of rehabilitative care across healthcare systems.

Her professional experience spans multiple outpatient clinics in New York City, including Manhattan, Queens, and the Bronx, where she assists in implementing individualized plans of care for patients with work-related and accident-related injuries. Her responsibilities include guiding patients through therapeutic exercises, applying physical therapy modalities, monitoring patient responses, documenting progress, and ensuring adherence to safety, compliance, and return-to-work protocols. Charmaine J. Garcia, PT

Charmaine J. Garcia is a licensed Physical Therapist with extensive experience in outpatient orthopedic, sports, and neuromusculoskeletal rehabilitation. She provides individualized, evidence-based physical therapy care to patients recovering from injury, surgery, and movement-related conditions, with a strong emphasis on functional restoration, performance optimization, and patient education.

She earned her Bachelor of Science in Physical Therapy from De La Salle Health Sciences Institute in Cavite, Philippines. She brings over eight years of clinical and leadership experience across rehabilitation clinics, fitness-based therapy settings, and academic institutions, allowing her to integrate clinical precision with effective communication and team collaboration.

She currently practices as a full-time Physical Therapist in an outpatient rehabilitation clinic in New York, where she treats a broad range of neuromusculoskeletal, sports injury, and post-surgical cases. Her treatment approach incorporates therapeutic exercise, manual therapy, physical modalities, and structured progression based on patient goals and functional demands. Carlo Peña, PT, CSCS

Carlo Peña is a licensed Physical Therapist and Certified Strength and Conditioning Specialist (CSCS) with extensive experience in outpatient orthopedics, sports rehabilitation, and cash-based private practice. He specializes in helping athletes and active adults recover from injury, improve performance, and return safely to sport—with a particular focus on sprinting mechanics, deceleration control, and jump-based power training.

Carlo has treated a wide range of orthopedic and post-operative conditions in both clinic and private settings, and has worked with competitive soccer athletes through the MLS Next Academy at Metropolitan Oval. His background includes experience at Motion Sports Medicine, Fit Club Astoria, US Athletic Training Center, and as owner of a private PT practice. He has also rehabilitated Olympic weightlifters and high-performance athletes, integrating strength training with evidence-based rehab.

Carlo is currently completing his Transitional Doctorate in Physical Therapy (tDPT) at the University of Montana and holds a Bachelor of Science in Physical Therapy from the Philippines.

Jerome Rivera, PT, DPT

Dr. Jerome T. Rivera Jr. is a licensed Doctor of Physical Therapy specializing in outpatient orthopedic, neurological, cardiopulmonary, and geriatric rehabilitation. He is the founder of NYC Home Rehab Physical Therapy PLLC, a hybrid practice providing one-on-one physical therapy both in-clinic and through mobile in-home visits across New York City.

Dr. Rivera earned his Doctor of Physical Therapy from the University of Montana and graduated cum laude with a Bachelor of Science in Physical Therapy from the University of Santo Tomas in the Philippines. He is licensed in New York and previously practiced in the Philippines, where he gained clinical experience in hospital-based, outpatient, pediatric, and cardiac rehabilitation settings.

Before launching NYC Home Rehab Physical Therapy, Dr. Rivera served as a Director of Physical Therapy in an outpatient medical center, where he led a multidisciplinary rehab team and oversaw program development, patient outcomes, and clinical operations. His background includes:
